Work Environment

The Fairview Rehabilitation Center is an onsite facility located in the heart of Punta Gorda, Florida. Key Responsibilities

• Conduct thorough evaluations by reviewing patients' medical histories and performing detailed assessments to create individualized treatment plans.
• Communicate evaluation findings and craft treatment strategies in collaboration with patients and their families, ensuring they are involved in goal setting.
• Implement evidence-based treatment protocols, adapting to each patient's needs and progress, and consistently monitor their health status.
• Collaborate with multidisciplinary teams, including physicians and occupational therapists, to ensure cohesive care and patient safety.
• Document all patient interactions accurately and promptly, including initial assessments, daily notes, and discharge summaries.
• Participate in discharge planning, coordinating with care teams to provide seamless transitions for patients returning home.
• Manage your schedule effectively to meet productivity targets while maintaining high standards of care.
• Engage in local marketing initiatives to promote the facility and its services, enhancing community outreach.
• Stay current with best practices and new techniques through continuous professional education and serving as a clinical instructor for physical therapy students.

Patient Population / Client Focus

Qualifications

• Bachelor's Degree in Physical Therapy is required. A Doctorate in Physical Therapy is preferred.
• A valid state license for physical therapy practice required for Florida.
• Current basic life support (BLS) certification from the American Heart Association is necessary.
• Prior work experience is not required, making this an excellent opportunity for recent graduates.

Preferred Experience

• Experience in an outpatient setting, particularly in orthopedic rehabilitation.
• Familiarity with electronic health record (EHR) systems to manage documentation efficiently.
• Prior experience in collaborative healthcare environments enhancing patient-centered care approaches.
