RN Clinical Team Manager – Home Health Full Time, Monday‑Friday 8:00am–5:00pm (onsite). Elara Caring is seeking an experienced Registered Nurse to oversee and coordinate home‑health services for our patients and care teams. Responsibilities Provide oversight of all patient care services and personnel, including patient and personnel assignments, coordination of care, referrals, and continuous assessment of patient needs. Plan, direct, and participate in the delivery of diagnosis‑specific home‑health services, coordinating services with providers, vendors, or facilities. Ensure the timely start of patient care within recommended regulatory guidelines. Process and coordinate physician orders, staff assignments, and communication among direct‑care team members, patients and families, and physicians. Review and approve plans of care and evaluate proposed changes for clinical appropriateness. Integrate orders from all relevant physicians and ensure approval by the responsible physician. Conduct regular OASIS reviews for all patients. Meet with direct‑care team members regularly to provide guidance, coaching, and communicate information related to specific issues or organizational changes. Initiate hiring requests, interview candidates, and recommend hiring of direct‑care staff. Communicate frequently with the Branch Director or Administrator on key performance metrics. Remain available for field work as needed. Perform other professional duties or projects as assigned. Work in a physically demanding, high‑stress environment, frequently lifting, pushing, pulling, and supporting up to 50pounds. Qualifications Associate degree in a nursing‑related field. At least 2 years of registered‑nurse experience in home‑health or hospice settings. Preferably 1 year of supervisory experience. Current, unrestricted RN license valid in the state of work. Proficiency in enterprise applications such as Homecare Homebase, Workday, and Microsoft Office. Willingness to travel within the branch/office coverage area and possession of a dependable vehicle, valid driver’s license, and current auto insurance. Ability to frequently lift, push, pull, and support up to 50pounds, including positioning or transferring patients and moving equipment.
