Job DescriptionCompany Description RespectADHC is an adult day health care center dedicated to supporting the well-being, independence, and dignity of adults who require daily assistance and supervision. The organization provides structured programs, health monitoring, and social services designed to help participants remain in their communities as long as safely possible. RespectADHC works closely with families, caregivers, and healthcare professionals to coordinate comprehensive, person-centered care. The team values respect, cultural sensitivity, and collaboration in delivering services that promote quality of life and meaningful engagement for all participants.\nRole Description The Social Worker at RespectADHC is a Part-time, on-site role based in Winchester, MA. The Social Worker provides psychosocial support and case management for program participants and their families, conducts intake assessments, and develops individualized care plans in collaboration with the interdisciplinary team. Daily responsibilities include coordinating services, advocating for participant needs, facilitating groups and counseling sessions, and supporting transitions between care settings. The Social Worker maintains accurate documentation, participates in care conferences, and helps ensure regulatory and ethical standards are met. This role also involves building strong relationships with community resources and contributing to a safe, inclusive, and supportive environment for participants and staff.\nQualifications\n\n Relevant social work education (license: LCSW/LCISW) and training, such as a Bachelor’s or Master’s degree in Social Work or a related human services field\n Experience in adult day health care, gerontology, community-based services, or similar settings, with strong case management and counseling skills\n Knowledge of psychosocial assessment, care planning, crisis intervention, and community resource coordination\n Excellent communication, documentation, and interpersonal skills, including the ability to work effectively with diverse participants, families, and interdisciplinary team members\n Demonstrated commitment to person-centered care, cultural competence, ethical practice, and maintaining confidentiality\n Ability to work on-site in Winchester, MA, manage multiple priorities, and adapt to changing participant needs\n Familiarity with relevant state and federal regulations, and proficiency with basic computer applications and electronic record systems\n Licensure or eligibility for licensure as required by Massachusetts regulations for social work practice is preferred\n\ncontact: jessie thai View phone number on click.appcast.io or, View email address on click.appcast.io
