Humana Inc. (Inclusa) seeks a Care Coach I to evaluate member needs and guide families toward resources for optimal wellness. Remote, 40 hours weekly, with possible travel for training or meetings. You will coordinate services, monitor progress through assessments and active care planning, and ensure privacy and HIPAA compliance throughout care delivery.
