In this full-time, remote position, you will work primarily within our Detox Center to provide comprehensive psychosocial support to seniors and their families. This position offers the flexibility of remote work, allowing you to connect with clients from the comfort of your home or preferred location. While you will primarily be working with clients individually, you will also collaborate with interdisciplinary teams, including healthcare professionals and caregivers. You may occasionally meet clients in person at our Detox Center, where vital services are offered to seniors in need of specialized support.

Key Responsibilities\n

• Conduct in-depth assessments of seniors’ psychosocial needs, taking into account physical, emotional, and social dimensions to formulate personalized care strategies.

• Develop and implement care plans in partnership with clients and their families, ensuring that individual goals are prioritized and addressed.

• Provide individual and family counseling to help clients navigate various life transitions, challenges, and emotional struggles.

• Advocate for clients’ access to community resources, ensuring they receive appropriate services and support as necessary, particularly in crisis situations.

• Collaborate closely with nurses, healthcare providers, and other social workers to ensure a cohesive approach to client care and support.

• Regularly monitor clients' well-being through home visits or appointments, evaluate living conditions, and address potential safety concerns.

• Educate clients and families about relevant community resources, including support groups and programs that promote social engagement and improve overall quality of life.

• Maintain meticulous documentation of assessments, care plans, progress notes, and interactions to ensure continuity of care and compliance with regulatory standards.

Your primary focus will be on seniors and their families, particularly those experiencing the challenges associated with aging and detoxification processes. Understanding the unique psychosocial factors affecting this demographic is essential to tailoring effective interventions and support strategies. This includes addressing issues related to mental health, cognitive decline, social isolation, and access to essential resources.

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution.

• Current, valid state licensure as a Licensed Clinical Social Worker (LCSW).
