RN Case Management in Decatur, GA

Seeking experienced RNs for Case Management and Utilization Review roles, based in Decatur, GA. These contract positions offer an opportunity to apply your expertise in complex case management, focusing on patients with chronic conditions such as diabetes, cardiovascular issues, and behavioral health challenges.


Schedule/Hours:

• Day shift: 08:30 AM to 05:00 PM
• Rotating weekends (varies Sunday through Saturday)

Key Responsibilities:

• Manage complex cases involving acute and chronic conditions with a comprehensive approach.
• Conduct utilization review to ensure effective resource allocation.
• Collaborate closely with healthcare teams to coordinate patient care and discharge planning.
• Participate in interviews with the hiring manager as part of the selection process.

Qualifications and Desired Experience:

• Minimum five (5) years of clinical nursing experience in a hospital setting.
• At least five (5) years of acute care professional practice.
• Two (2) or more years of experience in utilization review, case management, and discharge planning are preferred.
• Bachelor's Degree in Nursing, Healthcare, or Master's degree in Case Management.
• Complex Case Management certification preferred.
• Alternatively, completion of a Master's degree in a Case Management Program with a minimum of two (2) years of RN experience in patient care delivery.
