Work Environment

Your role will primarily involve providing onsite care in the patient's home, ensuring they receive personalized treatment in a familiar and comfortable environment. Key Responsibilities

• Conduct thorough initial assessments of each patient, including reviewing medical histories, identifying functional limitations, and setting realistic goals for rehabilitation.
• Create individualized plans of care based on assessment findings that align with the patients' objectives and overall health improvements.
• Engage with patients to explain their treatment journey, emphasizing the importance of a collaborative approach to rehabilitation where patients actively commit to their care plans.
• Implement evidence-based interventions such as therapeutic exercises, education, and manual therapy techniques to facilitate healing and recovery.
• Continuously monitor patient progress and make necessary adjustments to treatment plans to ensure optimal outcomes.

Qualifications

• Possess a Doctoral Degree in Physical Therapy (DPT) from a reputable and accredited institution.
• Hold a current and valid license to practice as a Physical Therapist in the State of California.

• Have a minimum of 3 years of experience in orthopedic physical rehabilitation.

Compensation and Benefits
