Key Responsibilities\n

• Conduct thorough psychosocial assessments for patients in the hospice center to identify emotional, social, financial, and spiritual needs.

• Provide individual and group counseling sessions focusing on adjustment, grief, and coping mechanisms for patients and their families.

• Collaborate with healthcare teams to develop and implement holistic care plans that enhance the quality of life for patients.

• Coordinate with external community organizations and resources to facilitate post-discharge care and ensure support continuity.

• Offer emotional education and support regarding treatment options, available resources, and end-of-life planning.

• Ensure accurate and timely documentation of all patient interactions, maintaining compliance with legal requirements and hospital policies.

Qualifications\n

Compensation and Benefits\n

The salary range for this position is between USD 70,000 and 100,000 annually, commensurate with experience and qualifications.
