Key Responsibilities\n

• Conduct individual and group therapy sessions via secure video conferencing platforms, creating a supportive and engaging therapeutic space.

• Develop, implement, and monitor personalized treatment plans tailored to meet the unique psychological and emotional needs of each client, incorporating both short-term and long-term strategies.

• Work collaboratively with an interdisciplinary team that includes physicians, nurses, and other healthcare professionals within the hospice center, ensuring a holistic approach to patient care.

• Maintain accurate and confidential client records, adhering to HIPAA regulations and agency policies, thus ensuring continuity of care and upholding ethical standards.

• Facilitate psycho-educational workshops and support groups focusing on coping strategies, grief, and loss, aimed at empowering both patients and their families.

Patient Population / Client Focus\n

Your primary focus will be on clients within the hospice care setting, including patients experiencing terminal illnesses and their families. You will provide essential emotional support, assist with coping strategies related to grief and loss, and encourage conversations around advanced care planning and quality of life issues.

Qualifications\n

• Licensed Clinical Social Worker (LCSW) in the state of New York is mandatory.

• A Bachelor's degree in Social Work or relevant field.

• Proficiency in using telehealth technologies for delivering virtual care.

• Excellent verbal and written communication skills, combined with strong interpersonal abilities that enable you to connect meaningfully with clients from diverse backgrounds.

Preferred Experience\n

• At least two (2) years of experience in a clinical social work setting, especially within hospice or palliative care environments.

• Experience working with multicultural populations and understanding the nuances of different cultural beliefs surrounding death and dying.

Compensation and Benefits\n
