Important Requirements

• Minimum 2 years of transitional care experience in a hospital setting required.
• Minimum 2 years of hospital social work experience required.
• Flu vaccine required.
• Fingerprint cards required for background clearance within 1 week of offer.
• Must be at least 21 years of age.
• BLS preferred.
• Must maintain strict confidentiality of patient care information.
• Must maintain annual competencies.
• Must work collaboratively with multidisciplinary teams and state and community agencies.

Position Summary

The Clinical Social Worker provides integrated social services and care management to patients admitted to the hospital from birth through end of life. The role assesses biopsychosocial needs, supports discharge planning, identifies barriers and community resources, completes swing bed admission assessments, assists with financial, housing, legal, placement, and care needs, provides supportive counseling and advocacy, and coordinates services with patients, families, providers, multidisciplinary teams, and community agencies.

Key Responsibilities

• Assess the biopsychosocial needs of hospitalized patients.
• Develop discharge plans in collaboration with patients, families/caregivers, and care coordinators.
• Identify barriers to discharge, community resources, agencies, and DME needs.
• Complete swing bed assessments and patient disposition.
• Facilitate or co-facilitate patient and family care conferences.
• Provide guidance regarding acute and chronic illness, care plans, and healthcare system navigation.
• Assist patients with healthcare decisions, including Advance Directives, Power of Attorney, and POLST.
• Identify and coordinate community resources for housing, meals, transportation, and other basic needs.
• Assist with financial and legal matters and related applications or referrals.
• Assist with long-term care and assisted living placement, including applications, payer verification, transportation, and facility coordination.
• Identify alternative solutions to meet patients' physical and emotional needs.
• Coordinate dental, denture, podiatric, eye, and hearing services during hospitalization.
• Refer patients to appropriate social service clinicians for chemical dependence, mental health, and crisis support.
• Maintain complete and accurate records and timely patient documentation.
• Participate in multidisciplinary team meetings, staff meetings, and required training.
• Maintain knowledge and working relationships with community resources.
• Establish trusting relationships with assigned patients to support care and recovery.
• Support provider decision-making and patient-provider interventions.
• Coordinate with multidisciplinary team members to support continuity of care.
• Obtain patient information in a courteous, respectful, and culturally sensitive manner.
• Provide advocacy and outreach.
• Use conflict resolution and problem-solving skills to support patient care and team relationships.
• Coordinate with state and community agencies on behalf of patients.
• Work independently and manage multiple responsibilities in a fast-paced environment.

Required Qualifications

Education

• BSW or MSW in Social Services required.

Experience

• Minimum 2 years of transitional care experience in a hospital setting.
• Minimum 2 years of experience in social work in a hospital environment.

Preferred Qualifications

• BLS preferred.

Schedule

• Monday Friday, day shift.
• 5 8-hour shifts or 4 10-hour shifts.
