Complete swing bed admission assessments and ensure patients and families understand participation requirements for subacute rehabilitation services. Facilitate or co-facilitate patient/family care conferences to explain illness, plan of care, and navigate the healthcare system. Assist patients with financial, housing, transportation, and DME needs; guide applications and referrals to state, federal, and community resources. Identify custodial, legal, or advance directive considerations and refer to appropriate specialists for crisis or behavioral health support; maintain accurate documentation in the medical record.
