Advanced Health Care Clinic - Downey, South Gate, Bell, Lynwood, Los Angeles, Huntington Park, CA.

Primary Care Provider - Medical Clinic.

Part time or Full Time.

$61-94 per hour depending on years of Nurse Practitioner or Physician Assistant experience.

Position Overview:

• 2+ year of experience.
• Medical Spanish or Fluent Spanish.
• Family Nurse Practitioner (FNP).
• Certified Nurse practitioner (CNP).
• Physician Assistant (PA-C).

Full-time preferred. Part-time considered.
Outpatient. No hospital rounds. No overnight calls.

Responsibilities:

• Provide comprehensive primary care to adult and geriatric patients.
• Manage chronic conditions (HTN, DM, CHF, COPD, etc.).
• Conduct annual wellness visits.
• Meet quality metrics aligned with HEDIS/value-based contracts.
• Collaborate with supervising physician and care team.
• Document efficiently in EMR.

Ideal Candidate:

• 2+ years primary care experience.
• Comfortable in value-based/capitated model.
• Strong communication skills.
• Efficient but compassionate.
• Spanish speaking.
