Care Manager

Position Overview We are seeking a compassionate Caregiver to provide hands-on support and build meaningful relationships with senior residents. Key Responsibilities Assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, mobility, and personal care, following Individualized Service Plans (ISP), and documenting changes. Promoting safety, dignity, and independence is essential, along with engaging residents through conversation and activities. Additionally, the role involves supporting dining services, maintaining a clean environment, and communicating effectively with families and care team members. In Memory Care settings, creating enriching experiences for residents with memory loss is a vital part of the job.
