Work Environment

This position is onsite, engaging with patients in their homes as part of our concierge service, which emphasizes accessibility and a tailored care experience. Key Responsibilities

• Conduct thorough cardiovascular assessments, which include detailed patient histories and physical evaluations to accurately identify existing heart conditions.
• Educate patients about managing their cardiovascular health, including detailed discussions about medications, lifestyle modifications, and preventive care strategies tailored to their individual needs.
• Regularly monitor and document patient progress, making adjustments to treatment plans based on comprehensive evaluations and emerging data.
• Participate actively in both inpatient and outpatient care settings, including essential tasks such as follow-up consultations and discharge planning for optimal patient transitions.
• Stay abreast of the latest advancements in cardiology through continuous education and apply this knowledge in practice to provide evidence-based care.

Patient Population / Client Focus

You will work primarily with patients who are experiencing a variety of heart conditions, ranging from chronic diseases to acute cardiovascular events. The diverse patient demographic ensures that you will encounter different challenges and cases, allowing for significant professional growth and development in cardiology. Qualifications

• A Master's Degree in Nursing from an accredited institution is required.
• Current New York State licensure as a Nurse Practitioner (NP) is essential.
• At least two years of relevant experience in cardiology or a closely related field, where you've successfully managed cardiovascular health.
• Exceptional clinical skills and proficiency in cardiovascular medicine, with a focus on providing compassionate and comprehensive care.

Preferred Experience

• Experience with concierge medicine or house call services is highly desirable.

Compensation and Benefits
