$300 sign on bonus** Lifespan Home Health is seeking a Care Team Assistant II for our Austin TX Agency. Essential Tasks Responsible for the receipt of home care referral and the scheduling of visits as prescribed in the plan of care. Will receive referrals and telephone/facsimile orders serving as the communication center between physician case manager, staff members, and client. Responsible for informing inquirers about the organization’s services and about home care in general. Schedules or reschedules visits when staff members are unable to make them as scheduled. Collaborates with payroll department to ensure that employee’s time records are accurate. Conducts client admissions/evaluations with the exception of clients in the PHC program. Submits required reports within the assigned time frame. Requirements High school diploma or GED required. Six months to one year previous experience in a home health agency or other medical setting preferred. Travel required for client visits/hourly care, as necessary.
