The requirements is for candidates to travel 50-75% of the time to meet with members face to face. This position holds a full caseload to manage waiver members. This position requires in person home visits with members. This position is critical to meet contractual requirements. Facilitate appropriate healthcare outcomes for waiver/LTSS members by providing care coordination, support and education for members throughRequired Qualifications:Minimum of two (2) years of case management experienceMicrosoft Office and electronic health record experienceMust possess reliable transportation and be willing and able to travel up to 75% of the time. Mileage is reimbursed by our company expense reimbursement policyValid Illinois Driver's license and must reside in one of the above zip codes or surrounding areas.Preferred Qualifications:Case management and discharge planning experienceManaged Care experienceMicrosoft Office experienceExperience working with HIV and AIDS populationMulti-lingual language proficiencyRequired Education:Individual with a bachelor's degree or a non-licensed individual with a master's degree, with either degree being in a human-services field (including, but not limited to sociology, special education, rehabilitation counseling)Anticipated Weekly Hours: 40Time Type: Full timePay Range: $21.10 - $44.99This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
