Clinical Social Worker, LCSW (Per Diem)

Utilizing clinical social work techniques and theory, consults on, assesses, and intervenes with high-risk patients and their families regarding the impact/amelioration of emotional and social problems. Duties also include, but are not limited to, conducting psychosocial assessments, making a psychosocial diagnosis, planning and implementing effective treatments, evaluation, education, supervision, coaching and mentoring of the patient (client) and family members. The position consults with, collaborates and communicates with a wide range of social, governmental and legal agencies, courts, schools, clinics, other hospitals, physicians and other sources in the overall care of the patient.

POSITION SUMMARY:

The Per Diem Inpatient Care Management Social Worker uses clinical social work techniques and theory to consult on, assess, and intervene with high-risk patients and their families on the impact and amelioration of emotional and social problems. Duties include, but are not limited to, psychosocial assessment, psychosocial diagnosis, treatment planning and implementation, evaluation, education, and the supervision, coaching, and mentoring of patients and family members. The position consults, collaborates, and communicates with a wide range of social, governmental, and legal agencies, courts, schools, clinics, other hospitals, physicians, and other sources involved in the patient's care.

• Shift requirement: Minimum of two shifts per month, 8:00 AM – 4:30 PM
• Weekend requirement: At least one of the two monthly shifts must be on a weekend.

ESSENTIAL RESPONSIBILITIES / DUTIES:

Assessment and Treatment Planning

• Assesses patients and families to identify emotional and social problems, using the department's psychosocial assessment tool as a guideline.
• Identifies the issues that must be addressed during the patient's care and formulates a plan.
• Identifies when the patient's family should be involved in the initial assessment and attempts contact the same day, if indicated.

Treatment and Patient/Family Support

• Provides psychotherapeutic treatment and/or psychosocial support to patients and families to reduce the impact of identified problems.
• Uses social work technique and theory to help patients and families cope with illness or social problems and improve functioning and emotional state.
• Keeps inpatients and families informed, involved, supported, and treated through at least weekly contacts. Increases contact whenever a change in care or plan requires it.

Protective Services

• Reports all suspected cases of abuse and neglect.
• Follows up to ensure the patient receives timely, comprehensive consultation and that legal mandates are met.

Interdisciplinary Collaboration

• Consults and collaborates with healthcare team members to ensure patients' emotional and social needs are addressed.
• Notifies supervisor when patients from an age group not regularly served are present. Seeks assistance from staff with knowledge of that age group's specialized needs.

Documentation and Confidentiality

• Maintains patient confidentiality according to hospital policy.
• Uses professional discretion when deciding what patient information to include in the medical record or department clinical notes.
• Notes in the medical record when critical patient information has been excluded and where it is available.
• Documents each patient encounter concurrently in the medical record or department clinical record according to department policy. Each entry includes date, time, title, and signature.
• Complies with departmental, regulatory, and professional requirements for documentation, recording, and data collection.
• Submits department statistics, records, and reports within required timeframes.

Supervision and Training

• Provides clinical supervision to other department personnel, as assigned.
• Supports graduate and undergraduate training programs by supervising MSW and BSW students, as assigned.

Department Operations and Quality

• Assists management in running a quality department by participating in department programs and following department policies and procedures.
• Identifies weaknesses in operations and recommends improvements.
• Serves on department committees as appointed by the Director.
• Represents the department throughout the hospital and in the community on department practice and the field of social work.

Professional Development

• Maintains competency in areas of responsibility to ensure quality care.
• Attends scheduled staff meetings and in-service education programs punctually and consistently.
• Obtains the continuing education units needed to maintain licensure.

Workplace, Safety, and Conduct

• Keeps work area and equipment neat and orderly. Helps clean and organize the department on own initiative and reports malfunctioning equipment to supervisor.
• Maintains a complete understanding of emergency procedures.
• Follows hospital infection control and safety procedures.
• Follows hospital standards of conduct, including those on patient rights, to provide the best possible customer service and patient care.
• Uses the hospital's behavioral standards as the basis for decision-making and to advance the hospital's mission.

Other Duties

• Performs other duties as needed.

JOB REQUIREMENTS

EDUCATION:

• Master's degree from an accredited school of social work required

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED :

• Active Massachusetts Licensed Certified Social Worker (LCSW) required

KNOWLEDGE AND SKILLS:

Required

• Knowledge of the social work process and of community resources and agencies related to health, illness, and disability, generally gained through one to two years of on-the-job training.
• Knowledge of community resources and agencies that serve children and families.
• Advanced interpersonal skills to: work effectively with a range of healthcare professionals, build and maintain therapeutic relationships with patients and families, and supervise graduate students.
• Ability to work on an interdisciplinary team.
• Comfort in a fast-paced, dynamic environment (a sense of humor is valued).
• Cultural sensitivity and comfort working with a wide range of racial and ethnic populations.
• Computer skills, including word processing, spreadsheets, and email (MS Word, Excel, Outlook) and web browsers.

Preferred

• Bicultural/bilingual competency.

SOCWKR

Compensation Range:

$34.81- $43.26
