PROGRAM: Assertive Community Treatment (ACT) - CCBHC
POSITION TITLE: ACT Program Director
REPORTS TO: Director of Clinical Services/CCBHC Program Director
JOB TYPE: Full-time
POSITION SUMMARY:
The Assertive Community Treatment (ACT) Director will launch and have direct oversight of the ACT Program, build the ACT staffing team, have responsibility for providing clinical, administrative, and operational leadership to multi-disciplinary teams, ensuring 24/7 care for individuals with severe mental illness for the Assertive Community Treatment (ACT) team within a Certified Community Behavioral Health Clinic (CCBHC) framework.
This position ensures delivery of comprehensive, person-centered, trauma-informed, and recovery-oriented services to adults with serious mental illness (SMI), consistent with:
- Illinois Administrative Code Title 59, Part 132 (Rule 132)
- Medicaid ACT service requirements
- CCBHC certification standards (SAMHSA criteria)
The ACT Program Director is responsible for maintaining ACT fidelity , ensuring Medicaid and CCBHC compliance , integrating care across systems, and achieving quality and outcome benchmarks required under the CCBHC model .
KEY RESPONSIBILITIES:
Program Leadership & Regulatory Compliance
- Direct ACT program operations in compliance with:
- Illinois Rule 132
- Medicaid ACT requirements
- CCBHC criteria and service scope
- Ensure program readiness for:
- State audits and certification reviews
- CCBHC site visits and reporting requirements
- ACT fidelity assessments
- Implement policies and procedures aligned with all regulatory frameworks
Clinical Oversight & Integrated Care
- Provide clinical supervision in accordance with Rule 132 and CCBHC standards
- Lead daily ACT team meetings and clinical staffings
- Oversee development and review of Individual Treatment Plans (ITPs)
- Ensure integration of:
- Mental health and substance use treatment
- Primary care coordination
- Care transitions (hospital discharge, crisis stabilization)
- Promote use of evidence-based practices, including:
- Integrated Dual Disorder Treatment (IDDT)
- Motivational Interviewing
- Trauma-informed care
CCBHC Service Integration
- Ensure ACT services align with the CCBHC required service array , including:
- Crisis services (24/7 access)
- Care coordination across physical and behavioral health
- Person- and family-centered treatment planning
- Collaborate with:
- Designated Collaborating Organizations (DCOs)
- Primary care providers
- Substance use treatment partners
Staffing & Workforce Requirements
- Ensure ACT team meets Rule 132 and CCBHC staffing requirements , including:
- Psychiatrist / APRN
- Registered Nurse
- Licensed Team Leader
- QMHPs
- Substance Use Specialist
- Vocational Specialist
- Peer Support Specialist(s)
- Maintain appropriate staff-to-client ratio (~1:10)
- Oversee hiring, credentialing, supervision, and workforce development
- Ensure staff meet cultural competence and trauma-informed care standards
Documentation & Medicaid Compliance
- Ensure all services meet Rule 132 documentation and medical necessity standards
- Monitor accuracy, timeliness, and compliance of clinical records
- Oversee Medicaid billing practices and encounter reporting
- Ensure alignment with CCBHC Prospective Payment System (PPS) documentation requirements
Quality Assurance & Performance Improvement
- Track and report on CCBHC quality measures , including:
- Hospital readmission rates
- Follow-up after hospitalization
- Engagement in services
- Substance use outcomes
- Monitor ACT fidelity using standardized tools (e.g., DACTS)
- Lead Continuous Quality Improvement (CQI) initiatives
- Use data dashboards to drive decision-making and program improvement
Financial & Administrative Oversight
- Monitor program budget and Medicaid/PPS revenue
- Ensure services align with financial sustainability and grant requirements
- Support reporting for state and federal funding sources
Community Partnerships & Care Coordination
- Develop and maintain partnerships with:
- Hospitals and health systems
- Federally Qualified Health Centers (FQHCs)
- Homeless service providers
- Criminal justice and crisis response systems
- Ensure effective care transitions and coordination across systems
Crisis Response & On-Call
- Ensure 24/7 crisis response capacity consistent with ACT and CCBHC standards
- Participate in leadership on-call rotation
- Provide clinical oversight during high-risk situations
QUALIFICATIONS:
Education
- Master’s degree in Social Work, Counseling, Psychology, or related field (required)
Licensure (Illinois)
- Active Illinois clinical license required (LCSW, LCPC, LMFT, or equivalent)
- Must meet Rule 132 Qualified Mental Health Professional (QMHP) criteria
- Eligible to provide clinical supervision
Experience
- 3–5+ years working with individuals with SMI
- 2+ years of supervisory or program leadership experience
- Experience with ACT, CST, or intensive community-based services required
- Experience in CCBHC or integrated care settings strongly preferred
- Experience with Rule 132 and Medicaid billing requirements strongly preferred
Knowledge & Skills
- Strong knowledge of:
- Illinois Rule 132
- Medicaid ACT services
- CCBHC certification criteria
- Experience with co-occurring disorders treatment
- Crisis intervention and risk management expertise
- Familiarity with PPS reimbursement and value-based care models
- Strong leadership, communication, and data-driven decision-making skills
