We are seeking an experienced and compassionate Director ofRehabilitation to join a Skilled Nursing Facility inConcord, California. This leadership role is responsible for overseeingrehabilitation services, supporting therapy staff, ensuring qualitypatient care, and driving positive clinical outcomes within ageriatric and post-acute care environment. The Director ofRehabilitation will work closely with interdisciplinary teams tomaintain operational efficiency, regulatory compliance, and highstandards of patient-centered care. Requirements
•Oversee daily rehabilitation operations within a Skilled NursingFacility setting
•Supervise and support therapy staff including PTs, PTAs, OTs,COTAs, and SLPs
•Ensure delivery of high-quality patient-centered care andcompliance with facility standards
•Coordinate staffing, scheduling, productivity, and programdevelopment initiatives
•Collaborate with interdisciplinary healthcare teams to supportpatient recovery and discharge planning
•Monitor clinical documentation, patient outcomes, and regulatory compliance
•Provide mentorship, training, and performance support forrehabilitation staff
•Assist with operational planning, quality improvementinitiatives, and department growth Benefits
•Active California license as a PT, OT, or SLP
•Previous rehabilitation leadership or supervisory experience preferred
•SNF, post-acute, or geriatric rehabilitation experience preferred
•Strong leadership, organizational, and communication skills
•Ability to manage multidisciplinary rehabilitation teams effectively
•Current BLS/CPR certification preferred
