Care Coordinator for High-Risk Patients (Bilingual)

Suvida Healthcare is seeking a Care Coordinator to support high‑risk patients under the supervision of the Nurse Care Manager. You will perform daily patient follow‑ups, manage DME and home health orders, triage calls, retrieve hospital records, and handle clerical tasks in the Chronic Care and Transition of Care programs. Ideal candidates have 2+ years as Medical Assistant or LPN/LVN, bilingual English/Spanish, strong communication, and proficiency with Microsoft Office; certification in #J-18808-Ljbffr Suvida Healthcare
