Position Overview

Position: Care Coordinator

Job Type: Full‑Time / Non‑Exempt

Shift Details: Rotating – 7:00 AM to 7:30 PM

Work Location: Crisis – Franklin County Crisis Care Center; Columbus, OH

Salary/Hourly Wage: $19.00 – $22.00 per hour

Essential Duties and Responsibilities

Guest Care & Advocacy

• Provide coordination of services and resources available internally and throughout the community.
• Act as an advocate for the needs and rights of individuals.
• Assist individuals in daily living activities and establish and maintain linkages that provide education, support and independence in the community.

Compliance & Risk Management

• Document relevant and required information and services to assure coordination of care as a communication tool with other team members.
• Keep abreast of all federal and state regulations and laws, and Recovery Innovations policies and procedures as they presently exist and as they change.
• Understand and adhere to Recovery Innovations compliance standards as outlined in the Code of Conduct and Conflict of Interest Policies.

Cross‑Functional Collaboration

• Coordinate or facilitate multidisciplinary team meetings to ensure services are provided in a timely manner.
• Partner with other provider agencies in the coordination of care to help participants achieve wellness.

Additional Duties

• Perform other related duties as assigned.

Qualifications

Education/Licenses

• Bachelor’s Degree in a behavioral health field and a minimum of one (1) year of behavioral health experience; or
• Bachelor’s Degree in any field and a minimum of two (2) years of behavioral health experience; or
• High School Diploma or equivalent and a minimum of four (4) years of behavioral health experience.

Experience

• Ability to provide courteous assistance in a concise, informative manner, keeping a positive tone and appropriate non‑verbal language.
• Work cooperatively in a team environment.

Skills

• Excellent computer skills, including Microsoft Office Suite.
• Demonstrated ability to effectively communicate both verbally and in writing.
• Excellent interpersonal, customer service and organizational skills with a high degree of accuracy.
• Must be able to pass de‑escalation training, as defined by state requirements.

Driving Requirements

• If driving is required for this position, must meet all requirements outlined in Recovery Innovations driving policy.
• If not qualified to drive per driving policy, a Non‑Driver Attestation form must be signed.

Miscellaneous Requirements

• Travel required based on business need.
• Applicants must be at least 18 years old unless residing in a state where funder or government regulations set a different minimum age requirement.
• Maintain a high level of confidentiality (HIPAA guidelines and regulations) and professionalism.
• We are a Second‑Chance Employer: must be able to pass a seven‑year background check, substance abuse testing and/or tuberculous testing (if applicable), education verification check, designated fingerprinting clearance in applicable states, three‑year motor vehicle report, and meet minimum eligibility requirements criteria of insurance carrier if driving is required.

Physical Demands and Work Environment

Work may be close to the eyes and involve substantial motions of the wrist, hands, and/or fingers. Frequent sitting, standing, walking, handling, finger dexterity; eye‑hand‑foot coordination, use of corrected vision; color vision; use of auditory senses. May be required to lift up to twenty (20) pounds. Work may occur in an office environment or behavioral health facility and may involve contact with individuals who are agitated and/or confused.
