Integrated Behavioral Health - LPCC or LISW - Part-Time - Mason Area

Job Responsibilities Psychosocial Assessment – Performs comprehensive assessments of complex social situations using advanced interviewing skills, intuitive knowledge and experience to identify challenges and issues associated with root causes. Provides appropriate interventions and treatment plans. Reports suspicions of abuse and neglect to legally mandated authorities. Completes documentation with accuracy and clarity. Resource Management – Serves as a leader in identifying and prioritizing interventions, anticipating and eliminating barriers, facilitating problem solving and conflict resolution, and empowering patients and families. Partners with the patient/family/caregivers to obtain financial assistance, community resources, or specialized equipment. Develops resource networks, excels at resource utilization, and acts as a resource to peers. Provides real‑time accurate information to patients/families. Advocates for patient population on a systems level by developing programs and protocols to better meet their needs. Educates patients and families to recognize progress and identify need for changes in treatment plan. Psychosocial Interventions – Provides psychosocial services as identified in the patient's comprehensive plan of care. Creates opportunities for and provides supportive counseling with the goal of maximizing emotional coping and adherence to the treatment plan. Facilitates collaboration with referral sources and health care team. Educates patients/families/team about evidence‑based treatment options. Provides self‑management support to high‑risk/complex patients/families. Identifies needs, develops programs, evaluates outcomes, and implements changes based on outcomes. Motivates and empowers patients/families/caregiver via anticipatory guidance. Initiates and facilitates family‑centered care team meetings. Demonstrates therapeutic approach focusing on micro and macro systems, assessment and crisis intervention, and problem prevention. Interdisciplinary Collaboration – Works for system improvement, promoting patient/family/caregiver well‑being. Advocates for patient population on a system level by developing programs and protocols. Serves as liaison between the patient/family/caregiver and medical team and outside agencies. Mediates as needed within the team on behalf of patient/family/caregiver. Empowers patients/families/caregivers to problem‑solve by exploring options and alternative plans. Develops processes for effective communication and coordination between team members while involving patient/family/caregiver in decision making. Collaboration – Partners with patient/family/team in safe transition of care to next level. Facilitates collaboration with referral sources, maintains knowledge of resources, develops networks, helps families become effective consumers, and advocates for patients at system level. Utilizes knowledge and experience in creating professional growth opportunities. Recognized as an expert by the health care team, serving as resource for educating professionals and students on psychosocial aspects of care. Assumes leadership role in improving division, teams, institution, and promotes a positive work environment.
