As a Healthcare Appointment/Transportation Coordinator at Apple Valley Village Health Care Center, you will schedule appointments/telehealth visits and corresponding transportation with community providers for facility residents on both TCU and LTC. Our ideal candidate has prior experience working in healthcare and/or patient scheduling. The ideal person for this role is organized, detail-oriented, and has excellent communication skills.

Position Type: Full-Time, benefits eligible position working Monday - Friday Shift Available: 8:00 AM - 4:30 PM Wage Range: $24.00 - $26.00 / hour depending on experience Location: 14650 Garrett Ave, Apple Valley, MN 55124

Healthcare Appointment/ Transportation Coordinator Responsibilities:

• Schedule appointments/telehealth visits and corresponding transportation with community providers for facility residents on both TCU and LTC.
• Provide excellent customer service to residents, family members and community providers.
• Assist with uploading documents into EHR.
• Fax census activity report to pharmacy.
• Complete discharge census and discharge summary observations.
• Perform additional tasks as needed.

Healthcare Appointment/ Transportation Coordinator Qualifications:

• Prior experience in healthcare and/or patient scheduling preferred.
• Must possess excellent customer service and communication skills.
• Ability to work independently and meet deadlines.
• Must be very detail oriented and able to work with multiple interruptions.
• Working knowledge of pertinent State and Federal regulations.
