Associate Center Clinical Director – Job Summary The Associate Center Clinical Director will directly supervise and train primary care providers (PCPs) in their assigned center. The incumbent is accountable for maximizing overall core model execution, including improving clinical quality, efficiency, outcomes, and clinician/patient satisfaction. The role includes direct patient care as a PCP and/or other clinical duties while the remainder of the time is devoted to leadership responsibilities such as PCP performance, engagement, and building a strong clinical‑operations synergy and culture. Time allocation depends on factors such as PCP capacity, market needs, center size, patient membership, and Market Clinical Director direction. Responsibilities Independently provide care for patients with acute and chronic illnesses encountered in older adult patients. Take full accountability for patient care and outcomes, appropriately seeking specialist consultation when needed, while remaining involved and responsible for detailed patient care. Engage with the hospitalist whenever one of their patients is in the hospital, regardless of hospitalist employment. Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs. Lead the care team comprising a care promoter (medical assistant), care facilitator, and care coordinator for patients able to visit the office. For patients unable to visit the office—whether in hospital, SNF, LTC, or homebound—engage with the transitional care team, case managers, acute and transitional‑care physicians, and other available resources as required. Lead weekly Super Huddle (SH) and Transforming Care Meeting (TCM); support the Center Manager (CM) or Center General Manager (CGM) and fulfill market clinical leadership duties as directed. Actively participate in center management, cover for other providers as needed, recruit and interview additional providers, and cover center clinical leadership gaps when required. Manage, mentor, and coach PCPs in assigned center to achieve outstanding clinical outcomes; include sampling other PCPs’ daily huddles within the center. Conduct leadership rounding with PCPs, reducing reliance on market clinical leadership. Partner with Center Operations Director/Market General Manager to improve center financial performance and increase center membership. Perform other duties as assigned and modified at manager’s discretion. Knowledge, Skills, and Abilities Proficient in Microsoft Office Suite products (Word, Excel, PowerPoint, Outlook) and other relevant applications. Willingness to travel locally as needed and, if applicable, nationally for initial training (2 weeks) and occasionally regionally and nationally for recruiting or training purposes. Fluency in English, both verbal and written; proficiency in other languages may be required depending on the center. Use and exercise of independent judgment in clinical and leadership duties. Education and Experience Criteria MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required. Current, active MD licensure in state of employment required. Minimum one year of clinical experience in geriatric, adult, or family practice setting preferred; Lead PCP ideally a ChenMed PCP Partner. Completion of Chen Medical training, including Masterful Conversations and meeting facilitation, as part of the individual development plan. Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty preferred; Board eligibility required. Once board certified, maintain certification in terminal specialty via MOC, CME, and/or retaking board exams as required. Current DEA number for schedule II‑V controlled substances required. Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within first 90 days. Pay Range $238,832 – $341,189 Salary. The posted pay range represents the base hourly rate or base annual full‑time salary for this position. Final compensation will depend on a variety of factors including but not limited to experience, education, geographic location, and other relevant factors. This position may also be eligible for bonuses or commissions.
