Description

• Assess patients in either an inpatient or outpatient context, developing tailored treatment plans
• Deliver physical therapy interventions centered on mobility, strength, balance, and functionality
• Collaborate with nursing staff, physicians, case managers, and other rehabilitation professionals to enhance outcomes
• Instruct patients and their families regarding therapeutic objectives, discharge planning, and home exercises
• Maintain documentation of evaluations, treatments, and patient progress following facility and regulatory standards
• Engage in care team rounds and interdisciplinary meetings to ensure cohesive care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited program

Certifications

• Must have state licensure as a Physical Therapist
• CPR certification is preferred
• Specialized certifications (e.g., in neurology or geriatrics) are advantageous but not mandatory

Skills

• Prior experience is highly desired
• Capability to treat patients with intricate medical and mobility challenges
• Excellent communication and teamwork abilities
• Proficient with digital charting and clinical documentation systems
