Role Responsibilities

Education Requirements

• MD or DO from a recognized medical institution.
• Completion of an accredited residency program in Family Medicine.

Certifications

• Board Certification in Family Medicine.
• Possess or be eligible for an active Arizona medical license.
• BLS and ACLS certifications are preferred.
