Clinical Social Worker II/III - Inpatient Psych

Job Overview Under the general supervision of the Director of Clinical Services and as part of a multidisciplinary treatment team, the inpatient social worker’s duties include but are not limited to psychosocial assessment, case management, individual and family therapy, and crisis intervention for patients hospitalized on an acute psychiatric unit. The social worker will provide timely and appropriate discharge planning taking into consideration the patient’s individual goals, preferences, needs and resources. Responsibilities Maintain a caseload of patients hospitalized on the acute psychiatric unit for adults and provide psychosocial assessment, case management, individual patient/family meetings, treatment planning, and discharge planning. Provide psycho‑education to patients and families. Communicate effectively and maintain positive relationships with community partners including SFDPH, Conservatorship Office, etc. Maintain up‑to‑date knowledge of the full spectrum of community resources, referral processes, and discharge planning needs, including basic needs (food, shelter), long‑term locked psychiatric facilities, residential care facilities, outpatient programs, clinics, and providers. Facilitate psycho‑education groups as needed. Provide clinical consultation to a multidisciplinary treatment team; facilitate referrals and services as needed; participate in ongoing assessment of treatment services, including evaluation of the need for change and improvements; participate in training/education of medical students, residents, and post‑graduate trainees. Provide individual psychotherapy, outreach, and case management as indicated by programmatic and patient‑care needs. Document patient care in a timely manner, in compliance with LPPH, TJC, Medicare, and private funding regulations/standards. Attend staff meetings and other duties as assigned; maintain knowledge of clinical service standards/regulations, infection control issues, national patient safety regulations; implement CDC hand‑washing guidelines; exercise sound judgment regarding sick leave to prevent illness spread; utilize LEAN Management systems, including huddle participation, True North Board leader rounding to address key performance indicators; participate in the facilitation of Patient Advisory or Focus Groups on behalf of LPPH. Minimum Requirements Master’s degree in social work, psychology, or a related field from an accredited university. For CSW2: eligible to register as an associate with the Board of Behavioral Sciences in the state of California. For CSW3: Licensed with the Board of Behavioral Sciences in the state of California (LCSW, LMFT). Minimum 3 years of relevant experience. Recent direct patient‑care experience in a mental‑health setting with group and crisis management skills. Demonstrated ability to provide comprehensive mental‑health services—including psychotherapy, crisis intervention, outreach, and case management—to a caseload of severely mentally ill adults who are high risk for psychiatric hospitalization. Demonstrated ability to complete psychosocial/functional assessments, mental status examinations, comprehensive treatment development, and discharge planning. Excellent documentation and clinical skills. Ability to prioritize, work independently, follow up efficiently, and work well under pressure and deadlines. Demonstrated excellent attendance and reliability. Demonstrated ability to assess suicide and violence risk. Demonstrated ability to work harmoniously with colleagues as a member of the treatment team. Excellent communication and interpersonal skills. A preliminary job offer and assignment may be extended pending successful background clearance. This position requires flexibility to orient and work at all UCSF Medical Center locations. Preferred Experience with populations diverse in ethnicity, gender, and sexual orientation. Experience with severely mentally ill patients.
