Registered Nurse, Case Managers – Home Health Care Old Glory Home Health Care & Assistive Care | Central Virginia

Our RNs provide skilled nursing visits, patient assessments, care coordination, medication education, wound care, disease management, post-hospital follow-up, and support for patients recovering from illness, surgery, injury, or a change in condition.

RN responsibilities may include:

• Skilled nursing visits in the patient’s home
• Start of care, recertification, resumption of care, and discharge assessments
• OASIS documentation and care planning
• Medication reconciliation and patient education
• Wound care and chronic disease management
• Communication with physicians, families, therapists, and the care team
• Vital sign monitoring
• IV Infusions
• Support for chronic disease management, including advanced heart failure, transplants, etc
• Documentation of patient progress and changes in condition
• Helping patients remain safe, independent, and supported at home

Requirements

• Active Virginia RN license
• Current CPR certification
• Reliable transportation
• Valid driver’s license and auto insurance
• Home health experience preferred
• OASIS experience preferred for RN applicants
