*Position Summary*

The Implementation Manager leads the onboarding of new client partners onto Vitasora Health’s care management platform, carrying each implementation from contract handoff through go-live and into a stable, fully supported state. This is a clinically grounded role. The Implementation Manager brings hands-on knowledge of EHR workflows and clinical operations to the design and configuration of each client’s program, translating how a practice actually documents, refers, schedules, and delivers care into a working setup. The role runs several implementations at once, coordinating internal teams, keeping client partners informed, and confirming that every configuration is accurate before launch. *Core Responsibilities*

*Client Implementation and Onboarding*

· Own the onboarding of new client partners, carrying each implementation from contract handoff through go-live and into a stable, fully supported state.

· Manage multiple concurrent implementations at different stages, tracking timelines, milestones, and dependencies across the full active portfolio.

· Serve as the main point of contact for client partners during onboarding, keeping them informed and supported throughout the process.

· Work cross-functionally with operations, IT, account management, and other internal teams to carry out the steps, handoffs, and dependencies each implementation requires.

· Follow the established implementation plan and process, coordinating with internal teams to keep each client’s onboarding on schedule.

· Identify roadblocks during onboarding and drive them to resolution, escalating to the right team or leadership when needed.

*Clinical Workflow Design and EHR Integration*

· Learn each client’s clinical workflows, including documentation, orders, referrals, scheduling, and care team roles, and translate them into a program design that fits how the practice actually operates.

· Serve as the clinical voice in each implementation, advising client teams on how remote patient monitoring, chronic care management, and telehealth workflows fit into existing care delivery.

· Coordinate EHR data flow and integration requirements with internal technical teams and client-side IT or EHR vendors.

· Map documentation, billing, and care team handoff points so clinical staff can adopt the program without disrupting practice operations.

· Identify clinical and operational workflow risks early and recommend practical adjustments before they reach the care team or the patient.

*System Configuration and Validation*

· Coordinate configuration of new client accounts within Vitasora Health systems, aligned to each client’s program design and clinical requirements.

· Validate that client setup, workflows, and data are accurate and aligned to program requirements ahead of launch.

· Partner with internal teams to support data integration, account structure, and platform readiness prior to go-live.

· Test and confirm system functionality end to end before each client launch.

*Training and Enablement*

· Train client-side clinical and administrative users on platform functionality, workflows, and best practices.

· Provide hands-on guidance and support to client teams during the transition to live operations.

*Go-Live and Stabilization*

· Lead go-live activities and provide close support through the first weeks of live operation.

· Monitor early program performance, resolve issues as they surface, and adjust workflows as the client team settles into the new process.

· Confirm the client is stable and self-sufficient, then hand off to account management and support with complete documentation of the setup.

*Documentation and Reporting*

· Maintain accurate, timely, and HIPAA-compliant documentation of implementation activity across all assigned clients.

· Ensure all client and patient information is handled in accordance with HIPAA standards and company confidentiality policies.

· Provide regular status updates and reporting to leadership on implementation progress across the active portfolio.

· Support continuous improvement of implementation playbooks, templates, and standard operating procedures.

This role may require completion of tasks and responsibilities outside of those listed above as business needs evolve. Flexibility and a willingness to contribute beyond defined expectations is an important part of this role.

*Required Qualifications*

· Active or inactive LPN or RN license

· Three to five years of experience in healthcare operations, clinical program delivery, or client implementation, including time in a client-facing or practice-facing role.

· Working knowledge of EHR workflows, including documentation, orders, referrals, scheduling, and care team coordination, and how clinical staff use them day to day.

· Demonstrated ability to manage several implementations or projects at once, at different stages, without losing track of details or deadlines.

· Experience configuring or supporting software platforms, ideally in healthcare, RPM, CCM, or telehealth.

· Strong relationship-building and organizational skills. Project coordination experience is helpful and can be developed in the role.

· Excellent written and verbal communication skills, with a client-focused approach and the ability to explain clinical and technical concepts to mixed audiences.

· Demonstrated attention to detail and a commitment to quality and accuracy.

· Proficiency with computers, common business software, and platform or CRM systems.

· Familiarity with HIPAA and a commitment to maintaining client and patient confidentiality.

· Ability to work independently in a fast-paced, remote environment.

*Preferred Qualifications*

· Experience in a remote patient monitoring, chronic care management, or telehealth setting.

· Experience with major EHR systems such as Epic, Cerner, athenahealth, eClinicalWorks, or NextGen.

· Prior experience as a clinical lead, charge nurse, or clinical supervisor in a practice or health system setting.

· Experience implementing or launching a new clinical program, service line, or technology at the practice level.

· Experience with care management platforms or similar clinical software.

· Bachelor’s degree in nursing or a related field, or equivalent experience.

*Schedule and Availability*

This role requires flexibility in scheduling based on business needs, which will be communicated in advance. Client implementations may require adjusting working hours, including occasional early mornings, evenings, or weekends, to meet go-live timelines and client needs. A willingness to work flexible hours is an important part of this role.

*Remote Work Requirements*

This is a fully remote position based in the United States. Team members are expected to maintain a reliable high-speed internet connection and a professional, distraction-free work environment, and to be available during core business hours to support client engagements and internal coordination.
