About the Position

Job Title: ECMO Specialist RN Profession: Registered Nurse Specialty: ECMO Specialist Duration: 13 weeks Shift: Night 4x12-Hour Hours per Shift: 12 Experience: 2 years minimum of ECMO Specialist experience in circuit management. 3 or more years of CVICU/ICU preferred. License: Active RN license Certifications: BLS, ACLS, NIHSS Must-Have: ECMO trained Description: The ECMO Specialist RN must have a minimum of two years of ECMO Specialist experience focusing on circuit management. Three or more years in CVICU/ICU is preferred, as the specialist may be floated to CICU/CVICU as needed. The required certifications include BLS, ACLS, and NIHSS. Preferred certifications are CES-A or E-AEC and LVAD. This position is not open to Respiratory Therapy or Perfusion ECMO Specialists. The assignment duration is initially for 13 weeks. Skills required include: Activated Clotting Time (ACT) management. Acute Kidney Injury care. Arterial line management. Assisting with chest tube insertion/removal. BiPAP/CPAP management. Blood product administration. Care of the patient on ECMO. Care of ventilated patients. Central line care and management. Central Venous Pressure Monitoring (CVP). Chronic/Acute renal failure management. Continuous IV paralytics/neuromuscular blockade management. Continuous IV sedation. Coronary Artery Bypass Graft (CABG) care. CVA/acute stroke management. CVVHD/CRRT knowledge. Management of emergency medications. Care for epicardial pacing wires. Management of external ventricular drains (EVD). Fibrinogen management. FloTrac/Vigileo monitoring. Care for fresh tracheostomies. Heart transplant experience. Hypothermia protocol management. ICP monitoring. Impella experience. Infection prevention skills. INR monitoring. Intake and output monitoring. Interpretation of arterial blood gases (ABGs). Dysrhythmia management. Intra-aortic balloon pump (IABP) knowledge. Isolation precautions and PPE knowledge. Management of liver failure. Lumbar drain experience. Management of high-output drains. Caring for patients with multisystem organ dysfunction/failure. Management of massive transfusion protocols. Monitoring for abdominal compartment syndrome. Nurse-driven ECMO protocols knowledge. Obtaining 12 lead ECG/EKG. Phlebotomy skills. PICC line management. Post-cardiac surgery care. Post-craniotomy care. Pre/post cardiac catheterization/intervention care. Procedural sedation administration/monitoring. PT/PTT monitoring. Pulmonary artery line/Swan Ganz monitoring. Pulmonary edema management. Pulmonary embolism management. Respiratory failure management. Sheath removal skills. Starting and maintaining IVs. Suctioning (ETT, NT). Titrating cardiac and vasoactive drips. Managing TPN/Lipids. Transcatheter Aortic Valve Replacement (TAVR) experience. Management of traumatic brain injury. Performing urine output assessments. Valve repair/replacement knowledge. Ventilator management skills. Experience with ventricular assist devices (LVAD/RVAD). ECMO specialist-specific training is required. Interdisciplinary support includes IV Teams, Physical Therapy, Respiratory Services, Interpretation Services, Radiology, Social Services, Rapid Response Teams, Pharmacy, Transportation, and Unit Secretaries. Patient diagnoses may include acute cardiac/respiratory failure, acute respiratory distress syndrome, cardiogenic shock, post-cardiotomy shock, and cardiac surgery. Special procedures in this role may include ECMO (VA, VV, V-PA), Impella (CP, 5.5, RP Flex), and IABP. Special equipment used may include CentriMag, CardioHelp, Impella, Teleflex IABP, Philips Monitors, Aquadex, PrismaFlex CRRT, Alaris IV pumps, Arctic Sun, ThermoGuard, and CardioQuip temperature heater/coolers.
