Work Environment

The role is based onsite within our dedicated rehabilitation facility in Perth Amboy, NJ, where you will engage directly with patients in a warm and welcoming environment. You will collaborate with a multidisciplinary team of healthcare professionals, working in proximity to nursing staff, occupational therapists, and administrative personnel to ensure seamless patient care.

Key Responsibilities

• Conduct comprehensive examinations and evaluations of patients experiencing functional limitations and disabilities, tailoring treatment plans that address individual needs.

• Implement and manage treatment protocols based on your clinical findings, adapting them in response to patient progress and evolving healthcare standards.

• Perform specific assessments to generate detailed documentation, including initial evaluations, ongoing progress notes, and discharge plans, documenting all relevant findings and treatment adjustments accurately.

• Engage in regular consultation with other healthcare professionals to coordinate integrated patient care and modifications to treatment plans as needed.

• Actively monitor and document changes in patients' conditions, modifying treatment plans and therapy goals to adapt to progressive recovery needs.

• Facilitate discharge planning by providing comprehensive follow-up care instructions, ensuring a smooth transition for patients after completing therapy.

• Ensure compliance with state regulations and company practices by partaking in necessary training and participating in facility meetings aimed at improving service delivery.

Patient Population / Client Focus

Your primary focus will be on individuals recovering from orthopedic surgeries, sports injuries, and musculoskeletal conditions. The role requires a genuine commitment to evaluate and treat patients both young and old, empowering them to regain independence and functionality through skilled therapeutic interventions.

Qualifications

• Valid and active state licensure as a Physical Therapist (PT) from the Physical Therapy Examining Committee.

• A Doctorate of Physical Therapy (DPT) from an accredited physical therapy program.

• A minimum of two years of outpatient orthopedic experience, demonstrating a strong foundation in rehabilitation practices.

• Current CPR Certification, ensuring preparedness for any critical situations.

Preferred Experience

• Experience with electronic medical record (EMR) systems to streamline patient documentation and improve communication across the care team.

• Prior involvement in community outreach programs or initiatives that promote physical wellness and education.

• Advanced skills in manual therapy techniques and a keen understanding of evidence-based practices.
