About the position: This LPN is required to work Mon-Fri, 7a-3p This position is located at Middletown The LPN assists in maintaining the health and well-being of program participants by monitoring health issues and implementing policies, procedures and programs designed to promote health and enhance independence and quality of life, in keeping with Oak Hill’s mission. Attends to the health needs of residents/program participants by becoming familiar with and maintaining current knowledge of their health issues. Maintains resident/program participant health by contacting the physician in medical emergencies, writing an incident report, and keeping the Manager or Program Supervisor and Group Home Nurse informed. Facilitates resident/program participant care and provides awareness and access to important health related information by taking doctors' orders and recording the information on medication Kardex, medication cards, nursing notes, and in the shift to shift communication book. Provides information to ensure continuity in care and programming by communicating all medical concerns regarding residents/program participants to the incoming shift, verbally and in writing.
