Your next nursing opportunity starts here. We are seeking a dedicated Registered Nurse (RN) with a strong background in home health and OASIS documentation for a 13-week full-time contract. Schedule:

• 13-week contract, with possibility for extension
• Full-time, Monday through Friday
• Likely on-call responsibilities
• Structured daily visit expectations

Key Responsibilities:

• Deliver skilled in-home nursing care and interventions
• Perform comprehensive patient assessments with accurate and timely OASIS documentation
• Collaborate closely with physicians and interdisciplinary teams to develop individualized care plans
• Manage wound care, medication administration, patient education, and ongoing disease monitoring
• Keep the clinical team updated on changes and progress in patient condition

Preferred Qualifications:

• Active RN license
• At least one year of recent RN experience in home health
• Proven experience with OASIS documentation (required)
• Competency using HomeCare HomeBase (strongly preferred)
• Ability to consistently maintain a full-time visit schedule
• Reliable transportation and a valid drivers license
