Geriatric Care Manager

To support a growing practice dedicated to geriatric care management, the contract Geriatric Care Manager will provide ongoing support to older adults and their families, working remotely with a flexible schedule while managing a personalized client load. Key responsibilities Serve older adults and their families to ensure safe aging at home Manage a dedicated client load, providing real, ongoing support beyond traditional healthcare roles Utilize technology to enhance connections with families and streamline care management processes Required qualifications Experience in geriatric care management, hospice, palliative care, discharge planning, or hospital social work Possession of an LMSW, LCSW, or RN is a plus but not required Strong interest in fostering connections with older adults and their families Creative problem-solving skills and adaptability to varied daily tasks Tech-savvy with an eagerness to leverage evolving technologies in care management
