Location: Waterbury, CT & Surrounding Area
Schedule: Full-Time | Field-Based
Compensation: $106,000–$132,000 (Flexible Based on Experience)

Position Overview

Our client, a locally owned and operated home health agency , is seeking a Full-Time Physical Therapist to provide skilled in-home therapy services throughout Waterbury and surrounding communities.

This role offers the opportunity to work independently while remaining connected to a supportive interdisciplinary team. You'll help patients improve strength, mobility, balance, safety, and functional independence in the environment where those skills matter most—their own homes.

The ideal candidate is a compassionate, self-motivated PT who values clinical autonomy, meaningful patient relationships, and quality care .

Key Responsibilities

• Conduct comprehensive in-home physical therapy evaluations and reassessments.
• Develop individualized plans of care based on each patient's condition, functional needs, and goals.
• Provide skilled treatment focused on mobility, strength, balance, transfers, safety, and independence.
• Monitor patient progress and adjust treatment plans based on clinical findings and outcomes.
• Educate patients, families, and caregivers on therapeutic exercises, equipment use, fall prevention, and home safety.
• Recommend appropriate assistive devices and modifications to improve patient safety and function.
• Maintain accurate, timely, and compliant clinical documentation.
• Communicate patient progress and changes in condition with physicians and interdisciplinary care teams.
• Coordinate with nursing, occupational therapy, speech therapy, and other healthcare professionals as appropriate.
• Participate in care planning and quality improvement initiatives.
• Help identify opportunities to strengthen and grow the organization's home health therapy program.

Qualifications

• Active Connecticut Physical Therapist (PT) license in good standing.
• Graduate of an accredited physical therapy program.
• Strong clinical assessment and treatment planning skills.
• Excellent communication, organization, and documentation abilities.
• Ability to independently manage a patient caseload and daily schedule.
• Patient-centered approach with a commitment to providing high-quality care.
• Ability to collaborate effectively with an interdisciplinary healthcare team.
• Valid driver's license and reliable transportation.
• Ability to travel locally throughout Waterbury and surrounding communities .

Preferred Experience

• Previous home health physical therapy experience.
• Experience treating adult or geriatric patient populations.
• Program development or clinical improvement experience.
• Interest in mentoring other clinicians or pursuing future leadership opportunities.

Home Health Experience Not Required

Experienced Physical Therapists interested in transitioning into home health are encouraged to apply. Candidates with strong clinical skills, independence, organization, and a desire to develop their home health expertise will be considered.

Compensation

• $106,000–$132,000 annually.
