The East Portland/Fairview, Oregon Veterans Administration Community‑Based Outpatient Clinic seeks a full‑time Board‑Certified/Board‑Eligible Family Practice or Internal Medicine physician to serve as the Primary Care Clinical Practice Manager Physician within the Portland VA Medical Center. The provider will deliver independent patient management as part of an interdisciplinary team and will lead a Patient‑Aligned Care Team (PACT) that serves approximately 900 veterans. This role manages diagnosis, treatment, and follow‑up of acute and chronic conditions including diabetes, hypertension, COPD, arthritis, depression, heart failure, and chronic pain. The position is Monday‑through‑Friday with no weekends or holidays. Applicants must be board certified in Family Practice or Internal Medicine and demonstrate leadership, administrative, and educational capabilities. Candidates should be motivated to promote staff education, inter‑professional training, and professional growth. Work Schedule Full‑time, Monday‑Friday with minimal call for abnormal labs. Requirements U.S. Citizen. Current, full, and unrestricted license to practice medicine or surgery in the U.S. Board certification in Family Practice or Internal Medicine.
