Our focus is on maximizing health, function, and quality of life for our patients.Position OverviewDepartment: RehabilitationRole: LPNShift: 12-hour shifts from 6:30 PM to 7:00 AMWeekend Requirements: Every other weekend and holiday rotationsCall Requirements: YesKey ResponsibilitiesProvide direct care to patients with high acuity needs.Assist nursing staff and healthcare team members in providing optimal patient care.Respond promptly to call lights and support patient safety initiatives.Utilize mobility equipment and assist patients in their rehabilitation activities.Maintain accurate patient records through electronic health record (EHR) systems.QualificationsValid LPN license.Minimum of 13 weeks of experience working with EPIC EHR.Inpatient rehabilitation or Med-Surg experience preferred.Certifications: Current BLS certification.At least 1 year of nursing experience in a skilled nursing or similar environment.Unit InformationNumber of Beds: 49Patient Ratios: 1:7Equipment: Mobility aids, vital machines, EKG machines, computers for charting.Additional InformationMandatory 30-minute breaks for 12-hour shifts; 15-minute paid breaks are included.Team collaboration is key; nurses are expected to assist C.N.As and other staff as required.Attendance is strictly monitored as per internal policies.If you are passionate about making a difference in the lives of others through rehabilitation, we encourage you to apply for this rewarding LPN position.
