Since then, for 80 years, across 100 countries, World Relief has partnered with local churches and communities to build a world where families thrive and communities flourish.Today, organizational programming focuses on humanitarian and disaster response, community strengthening and resilience, and refugee & immigrant services and advocacy.POSITION SUMMARY:The Community Health Worker (CHW) plays a vital role in supporting California's Medi?Cal transformation under CalAIM (California Advancing and Innovating Medi?Cal) by providing relationship?based, whole?person support grounded in lived experience. As a trusted member of the community, the CHW builds meaningful relationships, delivers culturally responsive health education, supports care coordination, and helps individuals and families navigate health and social service systems, including CalAIM Enhanced Care Management and Community Supports. The CHW works collaboratively with the World Relief CalAIM team, healthcare providers, and community partners to reduce barriers to care, address social drivers of health, and empower clients and improve health outcomes for the communities they reflect and serve.ROLE & RESPONSIBILITIES:Outreach & EngagementActively locate, contact, and engage clients who have been identified as eligible for CHW services.Use culturally and linguistically appropriate outreach strategies to build trust and establish rapport.Conduct in-person meetings with clients in the community or at provider locations to assess their needs and preferences.Document all outreach and engagement efforts thoroughly and in a timely manner.Health Promotion & EducationUse motivational interviewing, trauma?informed, and harm?reduction approaches to support clients in accessing healthcare, understanding the healthcare system, engaging in their own care, and connecting to community resources that promote health and well?being.Lead or help facilitate community workshops and group activities on topics such as healthy relationships and communication, nutrition, stress management and mental health awareness, substance use and tobacco cessation, violence prevention and coping skills, child and adolescent immunizations, housing resources, and navigating the healthcare system and public benefits.Develop and distribute health promotion materials.Act as a liaison between healthcare, behavioral health, housing, and community?based providers to support coordinated and integrated services.Assist clients with scheduling appointments, completing applications, and arranging transportation as needed.Support clients in building daily living skills and long-term self-sufficiency.Coordination & ReferralsProvide trauma?informed care coordination and referrals, recognizing the impact of trauma and prioritizing safety, choice, trust, and empowerment in all client interactions.Connect clients to essential needs such as housing resources, food pantries, transportation, and public benefits.Facilitate access to CalAIM services for eligible clients, including Enhanced Care Management and Community Supports, and collaborate with the CalAIM team to support coordination of services once clients are enrolled.Conduct follow?up with clients and service providers to confirm services are received and identify and address barriers to access.Documentation & ComplianceMaintain accurate and up-to-date records of all client contact, care plans, eligibility documentation, and referrals in the EHR system.Ensure compliance with program requirements, HIPAA regulations, and agency confidentiality protocols.Provide timely and accurate documentation and information to support reporting for funders.Contribute to data quality assurance and continuous quality improvement.Collaboration & Program ParticipationParticipate in multidisciplinary team meetings, trainings, and case reviews.Work collaboratively with internal teams and departments.Contribute to continuous program development and quality improvement initiatives.JOB REQUIREMENTS:Mature and personal Christian faithCommitted to the mission, vision, and values of World ReliefDesire to serve and empower the Church to impact vulnerable communitiesAble to affirm and/or acknowledge World Reliefs Core Beliefs, Statement of Faith, Christian Identity and National Association of Evangelicals' For the Health of The Nation documentBachelor's degree in Social Work, Psychology, Nursing, or a related field; OR Associate's degree in Social Work, Psychology, or related field with at least 2 years of mental health or social services experience; OR equivalent combination of training and professional experience.Minimum of 2 years of experience in case management, care coordination, or related social services role.Demonstrated lived experience that reflects the community served, including shared cultural background, life experiences, socioeconomic conditions, or other circumstances that contribute to a strong understanding of community needs.Certified Community Health Worker (or willingness to obtain certification within a designated timeframe).Strong cultural competency and sensitivity to the needs of diverse populations.Experience working with immigrant or vulnerable populations, including individuals experiencing homelessness, serious mental illness, substance use disorder, justice involvement, or complex health conditions.Proficient verbal and written English communication skills.Bilingual or multilingual in a language commonly spoken in the client community (Dari, Pashto).Comfortable driving clients in a large passenger van.PREFERRED QUALIFICATIONS:Familiarity with Medi-Cal, CalAIM, and managed care systems preferred.Strong understanding of medical terminology and chronic health conditions.Knowledge of local housing resources, HMIS, Continuum of Care systems, and community-based services.Demonstrated ability to provide trauma-informed, client-centered, and strengths-based services.Excellent interpersonal, organizational, and written communication skills.Proficient in documentation and case noting in electronic health management systems.Ability to apply professional judgment to complex issues, manage multiple priorities, and work both independently and collaboratively.Excellent communication, interpersonal, and problem-solving skills.Excellent planning, organizing, prioritization, and time management skills.Able to work in a fast-paced, challenging, and multi-cultural environment.Ability to maintain positive, professional relationships with clients, colleagues, government agencies, and community partners.Demonstrated knowledge of community resources and local healthcare systems.$26.50 - $26.50 an hourWorld Relief offers a competitive benefits package and employee discount program for full-time, Regular, employees.World Relief is honored to be recognized with the Gold-level Cigna Healthy Workforce Designation for exceeding the core components of our well-being program including leadership and culture, program foundations and execution, and whole person health.Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.We are proud to be an EEO/AA employer M/F/D/V.
