Job Title Mobile Health Coordinator - Street Medicine Position Details Requisition Number: 43614 Employment Type: Full Time Division: ACCLaim Family Medicine Compensation Type: Hourly Job Category: Patient Care Support Hours Worked: M-F 8AM-5PM Location: True Worth Clinic Job Description The Mobile Health Coordinator - Acclaim will support the Acclaim Street Medicine Program and Intensive Primary Care by planning and executing community outreach initiatives for both programs. This position is a frontline public health worker who will act as a patient advocate and liaison to enhance collaboration between the health care system, social services, and the community partners. The Mobile Health Coordinator also builds individual and community capacity by increasing health knowledge and self‑sufficiency, which will lower health disparities in Tarrant County by providing access to services and creating an effective system of chronic disease management for underserved populations. This role requires a close understanding of the community served as well as JPS Health Network services and programs. The Mobile Health Coordinator is expected to drive his or her own vehicle for transportation. Typical Duties Develop partnerships within JPS Health Network, Acclaim and community organizations. Conduct intake interviews with patients, including enrolling and/or referring patients into programs and assisting patients with completing applications and registration forms; assist with eligibility determination, enrollment, and follow‑up with uninsured patients. Provide assistance with transportation resources as appropriate. Work collaboratively with other clinical personnel assigned to the same patient to coordinate care. Recruit participants and facilitate self‑management, nutrition, psychosocial support groups and physical activity sessions according to State approved curriculum. Conduct needs assessments, develop patient plans, conduct and/or navigate patients to appropriate interventions designed to improve access to health care and/or health status. Provide referral and follow‑up services or otherwise coordinate and proactively identify and enroll eligible individuals in federal, state, and local private or nonprofit health and human services programs. Provide teaching and education on direct services such as biometric screenings, first aid, health promotion, disease prevention, and self‑management. Provide culturally appropriate health education, information, care coordination and outreach in community‑based settings such as homes, schools, clinics, shelters, local businesses, and community centers. Lead interdepartmental and interagency meetings regarding high ED users and vulnerable outreach clients. Collect and monitor data on ED usage, service agency usage, etc. Identify and resolve issues that create barriers to care for specific individuals. Attend outreach events and communicate with community partners to plan, organize, and coordinate outreach activities. Train and coordinate staff, students, etc., and maintain a calendar of events. Prioritize patients for outreach. Monitor health needs and reinforce treatment regimens. Perform other related job duties as assigned. Qualifications Required Education and Experience: High School Diploma, GED or equivalent. 3+ years’ experience with community outreach and volunteer coordination. Previous experience with Electronic Medical Records such as EPIC. Understanding of the culture, language, socio‑economic status, and life experiences of the community served. Required Licensure/Certification/Specialized Training: Successful completion of DSHS Certified Community Health Worker Certification within 6 months of hire. Valid Texas Driver’s license. Preferred Education and Experience: Bachelor’s degree in healthcare, education, business, psychology, computer science or related field.
