Mobile Health Coordinator – Street Medicine Requisition Number: 43614 Employment Type: Full Time Division: ACCLAIM FAMILY MEDICINE Compensation Type: Hourly Job Category: Patient Care Support Hours Worked: M-F 8AM-5PM Location: True Worth Clinic Job Description The Mobile Health Coordinator supports the Acclaim Street Medicine Program and Intensive Primary Care by planning and executing community outreach initiatives. This frontline public health worker acts as a patient advocate and liaison, enhancing collaboration between the health care system, social services, and community partners. The Coordinator builds individual and community capacity, increases health knowledge, and fosters self‑sufficiency to reduce health disparities in Tarrant County. The role often involves driving personal vehicle to transport patients and participate in mobile unit operations. Typical Duties Develop and maintain partnerships within JPS Health Network, Acclaim, and community organizations. Conduct intake interviews; enroll and/or refer patients into programs and assist with application and registration forms. Provide transportation assistance and resources as appropriate. Collaborate with clinical personnel to coordinate care for patients. Recruit participants and facilitate self‑management, nutrition, psychosocial support groups, and physical activity sessions per state‑approved curriculum. Conduct needs assessments, develop patient plans, and navigate to interventions that improve access to health care. Provide referrals and follow‑up services; proactively enroll eligible individuals in federal, state, and local health and human services programs. Educate patients on biometric screenings, first aid, health promotion, disease prevention, and self‑management. Offer culturally appropriate health education and outreach in community-based settings such as homes, schools, clinics, shelters, and businesses. Lead interdepartmental and interagency meetings regarding high Emergency Department users and vulnerable outreach clients. Collect and monitor data on ED usage, service agency usage, etc. Identify and resolve barriers to care for individuals. Attend outreach events and coordinate outreach activities with community partners. Train and coordinate staff and students; maintain a calendar of events and prioritize patients for outreach. Monitor health needs and reinforce treatment regimens. Perform other related job duties as assigned. Qualifications Required Education and Experience High School Diploma, GED, or equivalent. 3+ years of experience with community outreach and volunteer coordination. Experience with Electronic Medical Records such as EPIC. Understanding of the culture, language, socioeconomic status, and life experiences of the community served. Required Licensure/Certification/Specialized Training Successful completion of DSHS Certified Community Health Worker Certification within 6 months of hire. Valid Texas Driver’s license. Preferred Education and Experience Bachelor’s degree in healthcare, education, business, psychology, computer science, or related field.
