RN Care Navigator LTSS & Waiver Coordination
Molina Healthcare
Molina Healthcare is seeking a Registered Nurse to join our care management team in Illinois. The RN will perform comprehensive member assessments, develop and monitor care plans, and coordinate LTSS and waiver services with multidisciplinary teams to achieve optimal outcomes.
The role requires 2+ years in healthcare, including care management, a valid RN license, and willingness to travel 25–40% locally. Excellent communication and organizational skills are essential. Equal opportunity employer.
