The Older Adult Coordinator (OAC) is responsible for addressing system-level challenges in service delivery for older adults with behavioral health disorders, and those at risk of experiencing an accidental overdose. The OAC will assist as needed with discharge, aftercare consultation, and technical assistance for older adults with serious mental illness (SMI) and or substance use disorder (SUD) who are or have been discharged from local hospitals, state psychiatric hospitals, residential SUD treatment programs and homeless shelters to less restrictive environments (Recovery Residences, assisted living facilities, nursing facilities, and family placements) in Baltimore City. The goal of this position is to assist with successful transition from restrictive care to community living settings, prevent and reduce overdoses among older adults through improved access to treatment and expansion of harm reduction practices. Essential Functions- Assist in discharge planning with hospitals and local community providers for consumers from Baltimore City and those who will live in the city.
- Provide case consultation, technical assistance, training, and overdose prevention support to:
- Community residential settings (including but not limited to families, Recovery Residences, Residential Rehabilitation Programs, Assisted Living Facilities, Nursing Homes, shelters, Baltimore City Senior Centers and Adult Medical Day programs.
- Identify and address system-level challenges in service delivery for older adults with behavioral health conditions by:
- Creating an action plan to reduce overdose among older adults in Baltimore City
- Assessing and developing community-based resources for older adults with mental health and SUD needs
- Providing strategic direction, data, and policy recommendations on a variety of initiatives, including harm reduction strategies.
- Participate in case conferencing and problem-solving complex consumer situations and system-level challenges with the Behavioral Health Administration, community, inpatient and long-term care providers, Adult Evaluation and Review Services (AERS), Adult Protective Service (APS), and Community Forensic Aftercare Program (CFAP). This includes facilitation of reconsideration of consumers who have been rejected by behavioral health providers and follow-up support of consumers placed in the community.
- Collaborate and partner with BHSB staff to elevate the work.
- Collaborate with City, State and local partners to address system challenges in services for older adults.
- Work effectively and collaboratively with others, both internally and externally to BHSB.
- Foster open and honest dialogue within and across teams and between colleagues who have different backgrounds. Engage in honest dialogue and navigate conflict in day-to-day interactions.

- Serve as part of BHSB's disaster response team as needed, in any capacity that is needed.
- To perform assigned work functions, this position and those it supervises may have access to protected health information (PHI). It is expected that the employee will operate from a place of integrity and assume an ethical obligation to protect the security and confidentiality of all PHI in accordance with BHSB's policies.
- An offer of employment for this position at BHSB is contingent upon the results of a thorough background check, including criminal history, which will require prior authorization from the candidate.
- Perform other duties as assigned.
- Master's Degree preferred
- 5 years' experience in inpatient and outpatient care with adult and geriatric consumers living with SMI and SUD.
- Licensed Social Worker preferred.
- Advanced psychiatric skills in assessment, treatment planning, and evaluation of outcome-based system measures.
- Professional communication and collaboration skills and ability to form strong working relationships across disciplines, settings, and situations.
- Expertise in clinical training within a variety of disciplines, levels of preparation, and settings.
- Responsiveness & initiative to facilitate system integration and expansion efforts.
- Written and oral communication skills and database management.
- Intermediate to advanced technical and analytical skills in word processing, spreadsheet, and other standard business software.
- Problem-solving skills.
- Cultural competence.
- Ability to interact in a hybrid/virtual environment.
- Proficient using Microsoft Office Suite Word, Excel, Outlook and PowerPoint.
Supervisory Requirements : This position has no supervisory responsibilities. Typical Physical/Mental Demands and Working Conditions This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. This is a role that requires walking, standing and sitting, as well as moving about the office which would require the ability to move files, carry office supplies, open filing cabinets and bend or stand as necessary. This role requires travel to other locations for meetings and other business-related needs. Ability to travel 100% of the time is required. It is important to have access to some form of transportation to travel to off-site meetings, conferences, and site visits. Salaries are based on the requirements of each job, are non-negotiable and step increases are offered at regular intervals. The starting hourly/ annualized salary rate for this position is $79,725. The benefits plan includes comprehensive medical, dental and vision coverage, employer paid life insurance, short-term and long-term disability, a 403(b) plan with employer contribution, a choice of 4 FSA plans, an employee paid legal benefit, generous PTO and 13 days paid holiday. In addition, medical plan participants receive an employer paid HRA accessed by a $5,000/$10,000 debit card to cover eligible deductible and copay expenses, depending on chosen coverage.
