Falls Village Rehabilitation & Wellness is seeking a compassionate and motivated Full-Time Physical Therapist (PT) to join our rehabilitation team in Cuyahoga Falls, Ohio.

What You’ll Do

• Evaluate residents and develop individualized physical therapy plans of care

• Provide skilled treatment focused on mobility, strength, balance, transfers, gait, safety, and functional independence

• Establish measurable goals and monitor resident progress

• Complete evaluations, progress reports, recertifications, and discharge documentation

• Collaborate with PTAs, OTs, COTAs, SLPs, nursing, physicians, and the interdisciplinary care team

• Provide education to residents, families, and caregivers

• Complete accurate and timely clinical documentation using Net Health

• Work within PointClickCare (PCC) as part of the interdisciplinary care process

• Help residents achieve their highest possible level of independence and quality of life

What We’re Looking For

• Current or eligible Ohio Physical Therapist license

• Graduate of an accredited Physical Therapy program

• Strong communication and clinical decision-making skills

• Compassionate, dependable, and team-oriented

• SNF/LTC experience is a plus, but not required

• Familiarity with Net Health and PCC is a plus

• New graduates are strongly encouraged to apply!
