Common conditions include diabetes, hypertension, and general preventive and wellness concerns. About 20% of visits are completed through telehealth, giving you a varied and modern outpatient workflow.

We're looking for a Nurse Practitioner with strong adult outpatient primary care experience, a clinician who is caring, diligent, coachable, and grounded, a team player who works well in a highly collaborative clinic environment, a provider comfortable managing both in-person and telehealth visits, interest in both holistic and traditional approaches to care, and experience with, or a sincere desire to learn, trigger point and joint injections.

Job duties include providing comprehensive outpatient primary care services for adult patients, seeing approximately 20-25 patients per day in a wellness clinic setting, performing complete physicals, conducting new patient visits, managing follow-up visits for lab review, chronic disease care, and medication checks, performing pre-operative physicals, evaluating and treating common adult primary care conditions, managing medication refills and ongoing medication-related follow-up, providing telehealth visits, performing trigger point injections and joint injections when trained and appropriate, accommodating occasional walk-in patients as needed, documenting all patient care accurately and efficiently in eClinicalWorks, collaborating closely with clinical and administrative staff to support excellent patient care, and participating in limited every-other-weekend call for urgent escalations only.

Required qualifications include an active Nurse Practitioner license in Arizona, an active DEA license, 3+ years of primary care experience, a strong adult outpatient clinical background, ability to manage a patient volume of 20-25 patients per day, comfort providing both in-person and telehealth care, strong teamwork and collaboration skills, a coachable approach and openness to feedback.

Preferred background includes experience with trigger point injections, experience with joint injections, interest in wellness-focused care models, comfort integrating holistic and traditional treatment approaches, experience working in a collaborative outpatient clinic environment.
