Description

• Conduct patient evaluations in either inpatient or outpatient settings and create tailored treatment plans
• Implement physical therapy interventions that emphasize mobility, strength, balance, and overall functionality
• Work jointly with nursing staff, physicians, case managers, and various rehab professionals to achieve optimal results
• Guide patients and their families regarding therapeutic objectives, discharge preparations, and home exercise routines
• Maintain comprehensive documentation of evaluations, treatments, and patient progress in line with facility and regulatory standards
• Engage in interdisciplinary team meetings and care rounds to promote coordinated patient care

Education

• Completed a Doctorate in Physical Therapy (DPT) or a comparable accredited degree

Certifications

• State licensure as a Physical Therapist
• Preferred: CPR certification
• Specialized certifications (i.e., neurological, geriatric) are beneficial, but not mandatory

Skills

• Prior experience is highly desirable
• Capability to assist patients facing complex medical and mobility challenges
• Strong communication and collaborative skills
• Proficient with digital patient record systems and clinical documentation
