Case Manager

The case manager assumes responsibility for an interdisciplinary process which assesses, plans, implements, monitors, and measures the effectiveness of interventions to meet patient's treatment and discharge needs, which would achieve optimal clinical and resource outcomes.**Education/Training:** Graduate of NLN Accredited School of Nursing, BSN preferred or Masters Degree from an accredited school of Social Work. Ability to read, write, and communicate in English.**Experience****:** Minimum of one (1) year experience in an acute care hospital. Strong clinical, organizational, and interpersonal skills.**Licensure, Registration or Certification**: Current New York State Licensure as a Registered Professional Nurse. New York State Licensure to practice Social Work in the State of New York within 6 months after hire date. Certification in Case Management required 18 months after eligibility.
