CORE RESPONSIBILITIES
- Triage patients who arrive at the VICTA offices for services, including registration if needed, documentation of consent to treatment, and managing flow to nursing, medical, and/or behavioral health services.
- Welcome new clients to the facility and provide orientation to services and responsibilities.
- Engage in personal goal setting and follow-up with clients to promote positive health outcomes; provide individual support, coaching, and care coordination.
- Establish and maintain communicative relationships with clients and their families.
- Utilize personal experience to help clients self-manage recovery and system navigation through coaching and role modeling.
- Provide practical help and support, advocacy, and care coordination in partnership with the individual and any care team members.
- Work closely and collaboratively with the interdisciplinary team to ensure clients have comprehensive, coordinated care plans; provide thorough reports and updates to medical providers on client progress.
- Identify health care trends in the community; continuously expand knowledge of available community resources and make appropriate recommendations to clinical staff.
- Educate individuals, small groups, and communities on how to use health and social service systems; plan and lead health education groups; conduct outreach, including presentations at local agencies and community events.
EDUCATION/EXPERIENCE
High school diploma or equivalent.
Lived experience with behavioral health challenges and/or substance use disorder.
completion of approved training program.
Certified as CPRS and/or CCHW by the RI Certification Board or eligible for certification.
Spanish fluency preferred but not required.
