Schedule Monday – Friday, 10:00 AM – 6:00 PM Summary Family Service League is seeking a full‑time Case Manager for a homeless shelter in Brentwood, NY. The Case Manager will conduct client assessments, develop Independent Living Plans, link clients with community-based service resources, and assist them in achieving self‑sufficiency through advocacy, counseling, and resource referrals. Meet with clients at least twice a week for face‑to‑face meetings. Develop Independent Living Plans for assigned families. Fill out Mental and/or Physical Impairment forms with Independent Living Plan Violations. Complete SPA applications for eligible clients. Collaborate with Housing Specialists to find permanent housing options. Conduct client safety inspections twice a week and address safety issues. Assist clients with unit cleanliness, hygiene, and daily living skills. Collaborate with the Office Manager on client shelter obligations. Work with clients to create a budget and identify needs and obligations. Review, complete, follow up, and submit FSL Client Incident Reports to the Compliance Supervisor. Maintain and update a resource database of social service organizations. Provide ongoing support and counseling for clients in reaching their goals. Foster inter‑agency collaboration for client long‑term stability. Maintain up‑to‑date documentation in the Homeless Management Information System (HMIS) and client files. Complete monthly reports promptly and submit documentation to DSS Housing Liaison. Attend scheduled case management team meetings, individual supervision, and trainings as required. Oversee compliance with shelter rules, regulations, and policies. Set and observe appropriate boundaries with clients; maintain confidentiality and HIPAA protocols. Provide assessment, crisis intervention, and individual, family, and group services to children, youth, and families. Advocate for client needs. Provide transportation as needed. Collaborate with a larger treatment or service coordination team within and outside the agency. Collaborate with Security to discuss and resolve client incidents. Read and understand the Policy and Procedure Manual. Respond to daily emails promptly. Ensure safe and continuous operation of the program; maintain presence during inclement weather and emergencies. Perform all other duties as assigned. Qualifications High school diploma required; bachelor’s degree in Human Services or related field preferred. Minimum of 2 years of experience in human services or related field preferred. Knowledge of the Suffolk County Department of Social Services preferred. Experience working with at‑risk populations preferred. Excellent interpersonal, verbal, and written communication skills required. Strong organizational and time‑management skills required. Proficient computer skills, including Microsoft Office required; experience with HMIS, Google Apps, and web‑based project management software preferred. Valid and clean NYS Driver’s License required.
