Kendal at Home is seeking a Care Coordinator to support members by identifying needs and creating personalized care plans. You will work with members, families, providers, and community resources to ensure optimal well-being, while collaborating with our 8-person team in Columbus and surrounding regions. You will remotely support members from your home office in the Columbus region, with in-person visits as needed.
