Role Overview: The Occupational Therapist in Home Health evaluates and addresses patients' functional status, activities of daily living, fine motor skills, and adaptive equipment needs. They perform assessments, develop care plans in collaboration with physicians, and provide evidence-based therapy within legal practice scope. Key Responsibilities Conduct patient assessments to determine functional ability and treatment needs. Evaluate patient responses, monitor changes, and document progress for CMS compliance. Develop and implement personalized therapy plans in coordination with the healthcare team. Perform home assessments, considering diverse patient backgrounds and conditions. Qualifications & Skills Current state licensure in Occupational Therapy. Valid driver's license, vehicle insurance, or access to reliable transportation.
